II.2.4)Description of the procurement:
The aim of the service is for people to ‘live well with dementia’ and to support the health and wellbeing of people with dementia and their carers (as far as is possible). The service will host or provide a range of preventative, health, social care, support and wellbeing services which are located in one ‘hub’ location but services are also available in community ‘spoke’ spaces across the Borough.
The focus will be on appropriate advice and support planning, peer support, social activities and education. This will assist people to maintain independence and remain in their own homes for longer. The service also aims to prevent or delay the requirement for adult social care services, such as residential care.
The service aims to be innovative, measuring preventative activity and demonstrating efficiencies, such as savings to adult social care, or the levering in of additional resources.
The service will also have a wider community role, addressing social isolation and stigma faced by people with dementia and their carers and supporting our plans to make Merton a dementia friendly community.
